We accept most insurance plans.
Charges
Unless previous arrangements have been made, payment is due at the time of service. We do accept Visa, Mastercard, American Express and Discover.
Co-Payment Billing Charge
Co-payments are required by your insurance company and are to be paid at the time of service. Due to the high cost of billing, there will be a $5 billing charge if co-payment is not made at the time of service.
No-shows
In order to provide timely care to our patients, we ask that you would call us if you are unable to keep your appointment. Beginning February 1st, 2013 a no-show fee of $50 will be charged (this is not covered by insurance).
