If your insurance plan or HMO requires a referral from this office to see another physician, specialist, or to receive a form of therapy, be sure to get the referral BEFORE your visit to that office. It takes us time to prepare referrals and most insurance plans are no longer accepting referrals made after a visit. If you are in need of a referral for an existing appointment, contact the office staff after 9AM.

In order to process a referral, we will need the following information:

• The name of the physician who will be seeing your child
• The time and date of the appointment
• Contact information for the office in which the child will be seen

Please provide at least four days notice prior to the appointment.
